HelloPharmacist Scorecard of Betaine Hydrochloride with Pepsin by Metabolic Maintenance
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains two active ingredients: betaine hydrochloride and pepsin. Betaine hydrochloride is a source of hydrochloric acid—a natural stomach acid that your body produces to help break down food.
Pepsin is a proteolytic enzyme (protein-digesting enzyme) that also supports digestion in the stomach. The capsule itself is made from vegetarian cellulose, and the product includes a small amount of L-leucine as an inactive ingredient.
Does it work?
Couldn't assess
The evidence for what these ingredients can do isn't established in the data we hold. Betaine hydrochloride has been studied for indigestion (dyspepsia), low thyroid function (hypothyroidism), diarrhea, rheumatoid arthritis, food allergies, and allergic rhinitis (hay fever), but the research available to us rates all of these as having insufficient reliable evidence.
That doesn't mean the product won't help—it means the quality or amount of research isn't strong enough to say one way or another.
How safe is it?
Well-documented data
Betaine hydrochloride is not suitable for everyone. It boosts stomach acid, so it can irritate your stomach and isn't safe if you have ulcers, gastritis, or true acid reflux (GERD).
The hydrochloric acid it produces could irritate existing ulcers or slow healing and may cause heartburn. Pepsin is generally well tolerated for most adults, though it may occasionally cause digestive upset or trigger allergic reactions in sensitive people.
In rare cases, occupational exposure to pepsin has caused allergic rhinitis (hay fever symptoms) or asthma—though this is not a typical risk from taking a supplement capsule. Betaine hydrochloride should be avoided during pregnancy and breastfeeding because safety data isn't available.
Pepsin should also be avoided during pregnancy unless prescribed, and during breastfeeding there isn't enough safety data, so use is not recommended without medical advice.
Meds to double-check
Minor interactions only
Before taking this product, double-check with your pharmacist or doctor if you use any of these: proton pump inhibitors (PPIs) like omeprazole, H2-blockers like famotidine or ranitidine, or antacids. Betaine hydrochloride can reduce how well these drugs work by increasing stomach acid.
All three interactions are Minor severity, but they're real.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Only minor medication interactions are documented, and safety information is well characterized.
This product is designed to support stomach acid and digestion, but it's not for everyone. If you take medication to reduce stomach acid—like a PPI, H2-blocker, or antacid—you'll need to check with your doctor or pharmacist first, because this product may weaken those drugs.
Also avoid it if you have ulcers, gastritis, or acid reflux. Talk with your own healthcare provider before starting, especially if you're pregnant, breastfeeding, or take any stomach medications.

Betaine Hydrochloride with Pepsin by Metabolic Maintenance Drug Interactions
HelloPharmacist Interaction Report
Betaine Hydrochloride with Pepsin by Metabolic Maintenance has documented interactions with three types of stomach medications.
Betaine hydrochloride boosts stomach acid, which can weaken the effects of acid-reducing drugs. The main concern is with proton pump inhibitors (PPIs)—medications like omeprazole that lower stomach acid for reflux or ulcers.
Taking this product alongside a PPI may reduce how well the PPI works.
Read the full breakdown — every affected drug type, severity by severity
H2-blockers (like famotidine) and antacids face the same issue—betaine hydrochloride's acid boost can interfere with them. All three of these interaction types carry Minor severity, meaning the effect is real but not immediately dangerous.
Pepsin, the second active ingredient, shows no documented interactions in our data. Altogether, these interactions span 36 individual medications.
If you take any stomach medication, use the search tool below to check your exact drug before starting this product.
