Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: blueberry extract powder in a 4:1 concentration, meaning the extract is made from four parts of blueberries reduced to one. The powder form makes it easy to mix into smoothies, yogurt, or water.
There are no inactive ingredients listed.
Does it work?
Couldn't assess
The evidence for blueberry extract is mixed. It's rated possibly ineffective for high blood pressure.
For age-related cognitive decline, anxiety, athletic performance, high cholesterol (hyperlipidemia), and cardiovascular disease, there isn't enough reliable evidence yet to know whether it works. Blueberries are nutritious whole foods, but whether this concentrated extract delivers benefits beyond the fresh fruit is still being studied.
How safe is it?
Well-documented data
Blueberry is generally well tolerated orally. The main side effects to watch for are constipation, diarrhea, nausea, and vomiting—especially with freeze-dried forms.
In one study, about a quarter of people taking 50 grams daily of freeze-dried blueberries dropped out in the first week due to stomach upset. Fresh blueberries are fine in pregnancy and while breastfeeding, but safety data on high-dose concentrated extracts like this during pregnancy and lactation isn't well established.
If you're pregnant or nursing, talk with your doctor or pharmacist about whether this product is right for you.
Meds to double-check
Minor interactions only
Check with your doctor or pharmacist before taking this if you're on antidiabetes drugs (insulin, metformin, or similar), since blueberry extract may lower blood sugar. You should also confirm it's safe if you take flurbiprofen (an NSAID) or buspirone (an anti-anxiety drug), as the extract may affect how your body processes them.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Only minor medication interactions are documented, and safety information is well characterized.
Blueberry extract may be worth considering if you're looking for an antioxidant supplement, though evidence for specific benefits is still limited. Be cautious if you take diabetes medications, flurbiprofen, or buspirone—check your exact medications with your doctor or pharmacist first.
Stomach upset is possible, especially if you take a large dose. Talk it over with your healthcare provider before you start.

HelloPharmacist Interaction Report
Blueberry 4:1 Extract Powder by TerraVita Premium Collection can interact with a few medication types.
The ingredient is blueberry extract, which has documented interactions.
Read the full breakdown — every affected drug type, severity by severity
The most significant concern is with antidiabetes drugs (insulin, metformin, sulfonylureas, and others). Animal and lab research suggests blueberry extract might lower blood sugar, so taking it with diabetes medications could theoretically increase your risk of low blood sugar (hypoglycemia).
If you take any antidiabetes medication, watch for signs like shakiness, sweating, or confusion, and talk with your doctor or pharmacist before adding this product.
Blueberry extract may also affect how your body metabolizes flurbiprofen (a nonsteroidal anti-inflammatory drug, or NSAID) and buspirone (an anti-anxiety medication). Lab studies show it can slow their breakdown, but human studies are limited.
Both interactions are rated Minor.
Altogether, these interactions span 88 individual medications. Use the interaction checker below with your exact medications to see if any apply to you.

Blueberry Extract, Powder
Antidiabetes Drugs
Theoretically, blueberries or blueberry leaf extracts might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal and in vitro research suggests that blueberry and/or blueberry leaf extracts can lower blood glucose levels.
Buspirone (Buspar)
Theoretically, blueberry juice might increase blood levels of buspirone.
In vitro research shows that blueberry juice can inhibit the metabolism of buspirone, possibly by inhibiting cytochrome P450 3A (CYP3A) enzymes. However, pharmacokinetic research in humans shows that drinking 300 mL of blueberry juice 30 minutes before taking buspirone hydrochloride 10 mg does not significantly affect the concentration or clearance of buspirone.
Flurbiprofen (Ansaid, Others)
Theoretically, blueberry juice might increase blood levels of flurbiprofen.
In vitro research shows that blueberry juice can inhibit the metabolism of flurbiprofen, possibly by inhibiting cytochrome P450 2C9 (CYP2C9) enzymes. However, pharmacokinetic research in humans shows that drinking 300 mL of blueberry juice 30 minutes before taking flurbiprofen 100 mg does not significantly affect the concentration or clearance of flurbiprofen.
