The capsule also contains inactive ingredients—hypromellose, water, stearic acid, magnesium stearate, and silica—which are common binders and fillers that help form and preserve the capsule.

L-carnitine is effective for treating L-carnitine deficiency itself. For other uses, the evidence is more mixed: it's considered possibly effective for high cholesterol (hyperlipidemia), congestive heart failure, and angina.

How safe is it?

L-carnitine is generally well tolerated at typical doses. The most common side effects are stomach-related—abdominal cramps and pain, diarrhea, heartburn, nausea, and reduced appetite—and some people report a fishy body odor.
Seizures are rare but have been reported. High doses are more likely to cause stomach upset.
There isn't enough reliable safety data for L-carnitine in pregnancy, so use only under medical supervision if you're pregnant. Safety while breastfeeding is not well studied, so caution is advised if you're nursing.

Formula
CarniteX, a highly stabilized form of the amino acid L-carnitine, supports cardiovascular health, pulmonary function, and muscle recovery from exercise. L-carnitine supports fat utilization and energy production in the mitochondria and is found in abundance in heart and skeletal muscle. CarniteX contains 60% pure L-carnitine and 32% natural L-tartaric acid.

The most serious concern is with blood thinners (anticoagulants): L-carnitine may increase the blood-thinning effects of warfarin (Coumadin) and acenocoumarol (Sintrom), a shorter-acting anticoagulant. There are at least two case reports of elevated INR (a measure of how thin your blood is) when these were used together, so if you take either of these medications, your doctor will need to monitor you closely.

L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
