Acetyl-L-carnitine is possibly effective for age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic neuropathy, and depression.

L-carnitine is generally well tolerated at typical doses, though high doses can cause stomach upset, diarrhea, nausea, heartburn, and a fishy-smelling body odor. Acetyl-L-carnitine is generally well tolerated short-term but may cause headache, insomnia, agitation, dry mouth, reduced appetite, nausea, vomiting, or diarrhea.
Seizures are a rare serious side effect of both forms. For pregnancy, there isn't enough safety data on L-carnitine—talk with your doctor or pharmacist.
Acetyl-L-carnitine is considered best avoided in pregnancy unless your doctor advises otherwise. For breastfeeding, L-carnitine is possibly safe but not well studied; acetyl-L-carnitine safety is unknown, so it's the safer choice to avoid it.

Mercola contains two active ingredients—L-carnitine and acetyl-L-carnitine—that carry documented interactions with blood thinners and thyroid medications. The most serious concern is with blood thinners (anticoagulants).
Both L-carnitine and acetyl-L-carnitine may strengthen the effects of warfarin (Coumadin) and acenocoumarol (Sintrom), which could increase your bleeding risk. There are at least two case reports of elevated INR (a measure of blood thickness) when L-carnitine was combined with acenocoumarol.

Both ingredients may also reduce how well thyroid hormone replacement works by blocking the hormone from entering cells. If you take thyroid medication, your dose might need adjustment.
Acetyl-L-carnitine carries an additional Moderate-severity interaction with serotonergic drugs—antidepressants and similar medications that affect serotonin. Acetyl-L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
