Taken the right way and for the short term, cascara sagrada is generally well tolerated. The most common side effect is mild belly discomfort and cramping.

Long-term use, though, carries real risks: it can cause low potassium, protein in the urine, blood in the urine, and severe weight loss (cachexia). Fresh or improperly aged cascara bark can trigger severe vomiting and, rarely, liver damage.

Cascara is not recommended during pregnancy — stimulant laxatives in this class are generally avoided then. While nursing, it's best to skip this supplement because the active compounds may pass into breast milk.
