Cassia seed extract is generally well tolerated in food amounts. At supplement doses — especially high doses taken long-term — there's concern about liver harm because of its coumarin content.
In one clinical trial, a rash appeared in one person taking 1 gram daily for 90 days; in rare cases, people with a cinnamon allergy have developed skin reactions after eating cinnamon in food. One person taking 1 gram daily for 3 months reported a low blood sugar event (hypoglycemic seizure), though it's unclear whether the supplement caused it.
Food amounts are likely safe during pregnancy and breastfeeding, but avoid medicinal or high-dose supplement amounts in pregnancy due to limited safety data, and avoid concentrated supplement doses while breastfeeding for the same reason.

The most serious concern is with drugs that stress your liver (hepatotoxic drugs). Large doses of cassia cinnamon taken long-term may add to that strain.
Cassia cinnamon contains coumarin, which can damage the liver in animal studies. In people, very high coumarin doses (50–7,000 mg daily) have caused liver damage that reversed when stopped; in clinical trials, doses of 360 mg to 12 grams daily for 3 months did not significantly raise liver enzyme levels, but the theoretical risk remains if you're taking other liver-stressing medications.
Cassia seed extract may also lower your blood sugar (glucose). If you take diabetes medications, it could add to their effect — potentially dropping your blood sugar too far.

Antidiabetes Drugs
Theoretically, cassia cinnamon may have additive effects with antidiabetes drugs.
Cassia cinnamon may lower blood glucose levels, and have additive effects in patients treated with antidiabetic agents. Dose adjustments to diabetes medications might be necessary.
Hepatotoxic Drugs
Theoretically, large doses of cassia cinnamon might cause additive effects when used with hepatotoxic drugs.
There is some concern that ingesting large amounts of cassia cinnamon for an extended duration might cause hepatotoxicity in some people. Cassia cinnamon contains coumarin, which can cause hepatotoxicity in animal models. In humans, very high doses of coumarin from 50-7000 mg/day can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.
