Cassia Cinnamon and Aprocitentan

Here is the interaction we found between these two, with the evidence behind it.

Moderate

HEPATOTOXIC DRUGS

Why this applies to Aprocitentan

Aprocitentan falls under HEPATOTOXIC DRUGS — the group Cassia Cinnamon interacts with.

Theoretically, large doses of cassia cinnamon might cause additive effects when used with hepatotoxic drugs.
There is some concern that ingesting large amounts of cassia cinnamon for an extended duration might cause hepatotoxicity in some people. Cassia cinnamon contains coumarin, which can cause hepatotoxicity in animal models. In humans, very high doses of coumarin from 50-7000 mg/day can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version. Aprocitentan (Tryvio) is a newer blood pressure medicine, and like others in its class it can occasionally put stress on your liver. Doctors usually check liver enzymes before and during treatment for that reason. Cassia cinnamon is worth a mention because it naturally contains coumarin, and in large, concentrated supplement doses taken over time, coumarin has been linked to liver irritation in sensitive people.

The concern here is simply additive: two things nudging the liver in the same direction. This is theoretical, based on how each behaves rather than proven cases, so please don't panic. Sprinkling cinnamon on your oatmeal is not the issue. If you take high-dose cinnamon capsules regularly, mention it to your pharmacist or doctor so your liver checks stay on track.

From our pharmacist’s overview on the Aprocitentan page

“Aprocitentan (Tryvio) is prescribed when blood pressure stays high despite other medicines, so anything that pushes your pressure around cuts straight to why you take it. Most of the 189 ingredients here do exactly that: some add to the drop and can leave you dizzy or faint, while others, tyramine among them, push pressure back up and…”

About Cassia Cinnamon

What is Cassia Cinnamon?

Cassia Cinnamon
  • Blood sugar support
  • Digestive upset
  • Antioxidant support
  • Flavoring and culinary use
  • Antimicrobial

Cassia cinnamon is the common, inexpensive cinnamon used in cooking, and it is also taken as a supplement, most often for blood sugar support. The evidence for its health benefits is mixed and far from proven, and because cassia is naturally high in coumarin, taking large amounts regularly may stress the liver. Use food amounts freely, but talk with a pharmacist or doctor before using high-dose supplements.

About Aprocitentan

What Is Aprocitentan Used For?

Brand name: Tryvio

Tryvio (aprocitentan) is used to treat hypertension (high blood pressure) in adults. It is always used together with other blood pressure medicines — not as a stand-alone treatment — and is specifically for people whose blood pressure remains too high despite being on other drugs.

Lowering blood pressure reduces the risk of serious cardiovascular events such as strokes and heart attacks. Tryvio is not a first-choice blood pressure medicine; it is intended for patients with hard-to-control high blood pressure who need an additional option.
