Cassia Cinnamon and Alogliptin, Pioglitazone

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Cassia Cinnamon interacts with whole categories of medication. Alogliptin, Pioglitazone falls into 2 of those categories, so it can interact with Cassia Cinnamon in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

HEPATOTOXIC DRUGS

1 of 2

Why this applies to Alogliptin, Pioglitazone

Alogliptin, Pioglitazone falls under HEPATOTOXIC DRUGS — the group Cassia Cinnamon interacts with.

Theoretically, large doses of cassia cinnamon might cause additive effects when used with hepatotoxic drugs.
There is some concern that ingesting large amounts of cassia cinnamon for an extended duration might cause hepatotoxicity in some people. Cassia cinnamon contains coumarin, which can cause hepatotoxicity in animal models. In humans, very high doses of coumarin from 50-7000 mg/day can result in hepatotoxicity that resolves when coumarin use is discontinued. Lower amounts might also cause liver problems in sensitive people, such as those with liver disease or those taking potentially hepatotoxic agents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the simple version. Oseni combines two diabetes medicines, alogliptin and pioglitazone. Pioglitazone in particular can, in rare cases, affect the liver, which is why your doctor may check your liver numbers from time to time. Cassia cinnamon naturally contains a compound called coumarin, and taking large supplement doses for a long stretch may also stress the liver in some people.

The worry is mostly theoretical and based on how these things work, not on strong human studies. Still, stacking two things that can each be hard on the liver isn't ideal. A sprinkle of cinnamon on your oatmeal is no problem. If you're thinking about high-dose cinnamon capsules, please run it by your pharmacist or doctor first so they can keep an eye on your liver.

ANTIDIABETES DRUGS

2 of 2

Alogliptin, Pioglitazone also falls under ANTIDIABETES DRUGS.

Theoretically, cassia cinnamon may have additive effects with antidiabetes drugs.
Cassia cinnamon may lower blood glucose levels, and have additive effects in patients treated with antidiabetic agents. Dose adjustments to diabetes medications might be necessary.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Oseni combines two diabetes medicines (alogliptin and pioglitazone) that work to lower your blood sugar. Cassia cinnamon, when taken in concentrated supplement doses, may nudge your blood sugar down a little too. Stacked on top of your medication, that means your numbers could drift lower than expected. The science behind cinnamon for blood sugar is mixed and not strong, so this isn't a guaranteed effect, but it's worth knowing.

If your sugar does dip too low, you might feel shaky, sweaty, dizzy, or unusually tired. The cinnamon you sprinkle on toast or oatmeal is very unlikely to matter. If you're thinking about a daily high-dose supplement, check with your pharmacist or doctor first so they can keep an eye on your readings.

From our pharmacist’s overview on the Alogliptin, Pioglitazone page

“Someone taking Oseni (alogliptin and pioglitazone) for type 2 diabetes often shops the very shelf this list comes from, and the pairing deserves genuine caution. Of the 356 ingredients on file, 239 also lower blood sugar, so stacking them on top of the pill can drop glucose too far.”

About Cassia Cinnamon

What is Cassia Cinnamon?

Cassia Cinnamon
  • Blood sugar support
  • Digestive upset
  • Antioxidant support
  • Flavoring and culinary use
  • Antimicrobial

Cassia cinnamon is the common, inexpensive cinnamon used in cooking, and it is also taken as a supplement, most often for blood sugar support. The evidence for its health benefits is mixed and far from proven, and because cassia is naturally high in coumarin, taking large amounts regularly may stress the liver. Use food amounts freely, but talk with a pharmacist or doctor before using high-dose supplements.
