Lactobacillus acidophilus is generally well tolerated in healthy adults and children. A small concern exists that live probiotic bacteria could, in rare cases, translocate from the gut and cause infection in vulnerable people — cases of bacteremia and other serious infections have been isolated and reported, though most happen in patients with underlying conditions.

Gastrointestinal side effects like stomach discomfort, gas, bloating, or diarrhea may occur when taking probiotics in combination, but they're uncommon. One clinical trial reported two rash cases and one case of itching, though it's unclear whether these were due to the L. acidophilus itself, other ingredients, or individual sensitivity.
