Andrographis is generally well tolerated when taken short-term, but long-term safety and quality of products aren't well established — that's something to keep in mind. Side effects are more likely at higher doses (5–10 mg/kg of andrographolide or above) or beyond 14 days of use.

Common ones include stomach discomfort, nausea, diarrhea, dizziness, fatigue, headache, taste changes, rash, itching, and hives. Rare but serious reactions include severe allergies and anaphylaxis.

However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models.
