Andrographis is generally well tolerated when taken short-term, though long-term safety and product quality aren't well established. Adverse effects are more likely at higher doses (5–10 mg/kg of andrographolide or more) or beyond 14 days of use.

Common side effects include abdominal discomfort, nausea, diarrhea, headache, fatigue, dizziness, rash, itching, and altered taste. Skin reactions—rashes, itching, hives, and in rare cases severe allergic reactions including anaphylaxis—have been reported.

During pregnancy, the safety data advises against use because of theoretical concerns and animal study findings about reproductive effects. There isn't enough safety information to use this while breastfeeding, so it's best to avoid it.

However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
