Lactobacillus acidophilus LA-14 is generally well tolerated in healthy adults and children. Since it contains live organisms, there's a theoretical concern about infection in some patients, though serious cases are rare.

Orally taken, it may cause uncommon gastrointestinal side effects like abdominal pain, bloating, flatulence, diarrhea, or vomiting. Fructooligosaccharides are generally well tolerated at food levels but can cause gas and bloating at higher doses; a review of clinical trials found doses of 2.5 to 15 grams daily for up to 8 weeks didn't increase these symptoms more than placebo, though supplement-level safety data are limited.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
