What’s inside
Full disclosure
This product contains one active ingredient: codonopsis root extract, a traditional herbal remedy. The extract is delivered in a liquid formulation with glycerin and water as inactive ingredients.
Does it work?
Couldn't assess
The evidence on codonopsis for most uses—including asthma, cancer, respiratory tract infections, upset stomach, and high cholesterol—is insufficient. We don't have reliable data to confirm whether it works for any of these conditions.
How safe is it?
Well-documented data
Codonopsis is generally well tolerated in traditional food-like amounts, though high-quality safety data are limited. Very large doses (30–60 grams) may cause chest pain, abnormal heart rhythm, throat pain, or loss of voice.
Allergic reactions, including severe ones like anaphylaxis and hives, have been reported; one case involved anaphylaxis triggered by 20 grams of codonopsis root. The safety data advises against use during pregnancy and breastfeeding unless your doctor advises otherwise.
Meds to double-check
Moderate interaction found
Check with your doctor or pharmacist before taking this product if you use blood thinners or antiplatelet drugs (Moderate interaction), the cancer drug abiraterone (Moderate interaction), or diabetes medications (Moderate interaction). Altogether, these interactions span 211 individual medications.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Codonopsis has a long history in traditional use, but modern evidence for its effectiveness is limited. If you take blood thinners, diabetes medications, or abiraterone, you'll want to talk with your doctor or pharmacist before starting this product.
Pregnant or breastfeeding people should avoid it. If you're interested in trying it, discuss it with your own healthcare provider first.

Codonopsis Alcohol Free by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Codonopsis Alcohol Free by Hawaii Pharm contains one active ingredient—codonopsis root extract—that interacts with a number of medications.
The most serious concern is with blood thinners and antiplatelet drugs (like warfarin, aspirin, and clopidogrel). Codonopsis may increase bleeding risk when combined with these medications.
A small clinical study in people with coronary heart disease found that codonopsis liquor reduced platelet clumping, though it didn't affect other clotting markers.
Read the full breakdown — every affected drug type, severity by severity
Codonopsis also interacts with the cancer drug abiraterone (Zytiga). Animal research suggests it may speed up how your body clears abiraterone, potentially lowering its effectiveness.
Additionally, codonopsis may raise the risk of low blood sugar (hypoglycemia) if you're taking diabetes medications, since the herb itself appears to have blood-sugar-lowering effects.
Additionally, check your specific medications with the search tool on this page to see if any are affected.

For each ingredient in Codonopsis Alcohol Free with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Codonopsis Root Extract, Dry
Abiraterone (Zytiga)
Theoretically, taking codonopsis root with abiraterone might reduce the levels and therapeutic effects of abiraterone.
Animal research in rats shows that intragastric administration of codonopsis root along with abiraterone every 2 days for 2 weeks seems to increase the clearance of abiraterone and reduce the overall exposure and time to maximum concentration. This interaction has not been reported in humans.
Anticoagulant/Antiplatelet Drugs
Theoretically, codonopsis liquor might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
A small clinical study in adults with coronary heart disease shows that consuming Codonopsis pilosula liquor for 4 weeks inhibits platelet aggregation but does not affect tissue-type plasminogen activator (t-PA) or plasminogen activator inhibitor (PAI).
Antidiabetes Drugs
Theoretically, codonopsis might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Laboratory and animal research suggest that codonopsis has antidiabetic effects.
