Gotu kola is generally well tolerated short-term, but quality evidence on long-term safety is limited and liver damage has been reported — four case reports exist, though whether gotu kola was the cause isn't certain. The most common side effects are stomach irritation and nausea when taken by mouth, and eczema when applied to skin.

Avoid gotu kola during pregnancy (not enough safety data) and breastfeeding. Manganese at normal dietary doses is safe, but high-dose supplements can be harmful — there's concern that high doses might cause nerve damage including Parkinson-like symptoms, and one severe case of irreversible Parkinson disease linked to 100 mg daily has been reported.

Prenatal vitamins and food amounts are fine; avoid extra high-dose supplements unless a clinician advises it. Butcher's broom is generally well tolerated short-term but has limited long-term safety data.

Common side effects include diarrhea, indigestion, nausea, and vomiting. Avoid during pregnancy and breastfeeding due to insufficient safety information.

Theoretically, the risk for manganese toxicity might increase when taken with antipsychotic drugs.
Hallucinations and behavioral changes have been reported in a patient with liver disease who was taking haloperidol and manganese. Researchers speculate that taking manganese along with haloperidol, phenothiazine-derivatives, or other antipsychotic medications might increase the risk of manganese toxicity in some patients.
