DHEA is generally well tolerated in typical short-term doses, but as a hormone it can cause side effects and requires medical guidance. The most common issues are acne (usually mild and dose-dependent), headache, insomnia, mood changes, and nausea.

In women, masculinizing effects—voice deepening, abnormal hair growth, irregular periods, oily skin—can occur. Men may experience aggression and breast enlargement.

There's also theoretical concern that long-term use might raise cancer risk, though this isn't proven. DHEA-induced mania has been reported, along with anxiety, irritability, and depression in some people taking it.
