Lactobacillus acidophilus is generally well tolerated in healthy adults and children. That said, because it contains live, active microorganisms, there's a theoretical risk — extremely rare but documented in medical literature — that in certain patients it could cause infection in the bloodstream or other serious sites; most of these cases involved people whose immune systems or gut barriers were already compromised.

Oral use with other probiotics may cause mild gastrointestinal side effects like bloating, flatulence, abdominal pain, or diarrhea, though these are uncommon. One clinical trial reported two rashes and one case of itching, but it's unclear whether those were due to L. acidophilus or the combination.
