L. acidophilus is generally well tolerated in healthy adults and children. In clinical trials, some people experienced mild gastrointestinal side effects — epigastric discomfort, abdominal pain, indigestion, gas, bloating, diarrhea, vomiting, or burping — though these are uncommon.

Because L. acidophilus preparations contain live organisms, there's a theoretical concern that they might cause infection in certain patients, though most reported cases appear to be due to bacteria moving from other parts of the body where they naturally live. Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
