Gut Health 365 contains 25 ingredients, most of them digestive enzymes that help break down food. The active enzymes include amylase (starch), papain and bromelain (protein), lipase (fat), lactase (milk sugar), protease (protein), and several others that target different carbohydrates and fiber.
You'll also find two live-organism probiotics — Bacillus subtilis and Bacillus coagulans — designed to support gut flora, plus phytase, a plant enzyme that may help nutrient absorption. The product also includes prebiotic ingredients like PreforPro and a dried yeast fermentate to feed beneficial bacteria.
Inactive ingredients (fillers and capsule material) are rice dextrin, hypromellose, water, and medium chain triglyceride.

Evidence for most of these ingredients is thin. Lactase is effective for lactose intolerance — that one's established.
Bacillus coagulans is possibly effective for constipation and irritable bowel syndrome (IBS), and Bacillus subtilis is possibly effective for antibiotic-associated diarrhea. For the other active ingredients — papain, bromelain, the other enzymes, and phytase — the evidence is insufficient to rate whether they work for their claimed purposes.

Most of these ingredients are generally well tolerated when used short-term at normal doses. Papain can irritate the digestive tract and cause allergic reactions in sensitive people; avoid it during pregnancy due to safety concerns, and avoid supplemental papain while breastfeeding.
Bromelain is generally well-tolerated short-term, but avoid it during pregnancy and breastfeeding due to lack of safety data. Protease, lipase, phytase, and the Bacillus probiotics are generally well-tolerated but should be avoided in pregnancy unless a doctor advises otherwise, and used cautiously while breastfeeding.
High doses of papain may cause serious effects like severe stomach inflammation or esophageal perforation. Lactase is likely safe in pregnancy.
People exposed to powdered forms of these enzymes occupationally have reported respiratory allergic reactions; if you develop breathing problems, throat itching, eye irritation, or other allergic symptoms, stop and seek help.

There's also a theoretical concern that bromelain could raise levels of tetracycline antibiotics, though a human study found no difference in actual drug levels.

Probiotic Blend
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
