Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: gynostemma leaf extract in dry form. The extract is suspended in a liquid base of glycerin and water (no alcohol).
Gynostemma, also called jiaogulan, is a plant used in traditional Asian medicine.
Does it work?
Moderate evidence
Gynostemma has possibly effective evidence for type 2 diabetes—clinical research shows it can lower blood glucose levels. For athletic performance, cardiovascular disease, cancer, respiratory tract infections, and fatty liver disease, the evidence we have is insufficient to rate whether it works.
How safe is it?
Well-documented data
Gynostemma is generally well tolerated when used in tea or supplement form for up to 4 months; long-term safety isn't well studied. The most common side effects are diarrhea and nausea.
Dry mouth, dizziness, and insomnia have also been reported. Do not use during pregnancy—safety hasn't been established and it may harm the developing baby.
Avoid while breastfeeding because there isn't enough safety information.
Meds to double-check
Moderate interaction found
Before you start, double-check this product with your doctor or pharmacist if you take blood thinners or antiplatelet drugs—gynostemma may increase bleeding risk. Also flag it if you're on immunosuppressants or diabetes medications, as gynostemma may interfere with both.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
If you have type 2 diabetes and want to explore gynostemma, talk with your doctor first—it lowers blood sugar and could work too well alongside your current medication. Anyone on blood thinners, immune-suppressing drugs, or diabetes medications should check their exact prescriptions with their own pharmacist or doctor before taking this product.
It's generally well tolerated for short-term use, but the long-term picture isn't clear.

Gynostemma Alcohol Free by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Gynostemma Alcohol Free by Hawaii Pharm contains one active ingredient—gynostemma leaf extract—that interacts with three important drug categories.
The most serious concern is with blood thinners (anticoagulants) and antiplatelet drugs: gynostemma may increase bleeding risk when combined with these medications because it appears to have blood-thinning effects itself.
Read the full breakdown — every affected drug type, severity by severity
Gynostemma also interacts with drugs that suppress the immune system (immunosuppressants). Because gynostemma can stimulate immune activity, it might reduce how well those medications work.
Additionally, if you take diabetes drugs, gynostemma may lower your blood sugar further and raise the risk of low blood sugar (hypoglycemia)—the ingredient has been shown to reduce blood glucose levels in clinical research.
Allogether, these interactions span 327 individual medications. Use the interaction checker on this page to search your exact prescriptions before you start.
