This capsule contains 4 active ingredients. Bilberry extract is included for its anthocyanidin content; gotu kola extract traditionally supports circulation and wound healing; horse chestnut seed extract contains esculin and is used for vein and circulation concerns; and collinsonia extract (also called stone root) is an herbal addition.
The capsule itself is made from gelatin and vegetable cellulose, with vegetable magnesium stearate and silica as inactive ingredients to help with processing and flow.
Does it work?
Moderate evidence
Evidence for most uses in this product is limited. Horse chestnut seed extract is possibly effective for chronic venous insufficiency (CVI)—a condition where veins have trouble returning blood from the legs.
Gotu kola is possibly effective for venous insufficiency and burns. Bilberry, however, is rated possibly ineffective for night vision, and the other claimed benefits—cognitive decline, age-related macular degeneration, and atherosclerosis—lack enough reliable evidence to rate.
For collinsonia extract, we hold no effectiveness data.
How safe is it?
Well-documented data
Bilberry fruit extract from food is generally well tolerated, though concentrated supplement doses lack long-term safety data; the most common side effects are dark-colored stools, flatulence, and mild stomach discomfort. Gotu kola is generally well tolerated short-term, but there have been rare case reports of liver toxicity; the most common side effects are stomach irritation and nausea.
Horse chestnut seed extract (properly processed to remove toxic compounds) is generally well tolerated, though dizziness, stomach upset, headache, and itching have been reported; raw seed or bark is toxic and should not be used. Collinsonia extract has little safety data; large amounts may cause intestinal irritation and stomach pain.
Pregnancy and breastfeeding safety varies: bilberry is likely safe in pregnancy, but gotu kola and horse chestnut should be avoided, and collinsonia safety is unknown—talk with your doctor or pharmacist if you're pregnant or nursing.

The most serious concern is a moderate-severity risk of bleeding: both bilberry and horse chestnut may increase bleeding risk if you're taking blood thinners (anticoagulants) or antiplatelet drugs like aspirin or clopidogrel (Plavix).

Bilberry also interacts with blood sugar drugs (antidiabetes medications), potentially lowering your blood glucose further and raising the risk of low blood sugar (hypoglycemia). Additionally, bilberry may affect how your body processes certain medications metabolized through the CYP2E1 liver pathway, and in vitro research suggests it might reduce the effectiveness of erlotinib (Tarceva), a cancer drug.
Gotu kola can add to the sedative effects of CNS depressants (such as benzodiazepines, opioids, and sleep aids) and theoretically poses an additive risk if you take drugs known to be toxic to the liver (hepatotoxic drugs).

Gotu Kola extract
Cns Depressants
Theoretically, taking gotu kola might increase the sedative effects of CNS depressants.
In vitro research suggests that gotu kola may have sedative effects via binding of GABA receptors.
Hepatotoxic Drugs
Theoretically, taking gotu kola with hepatotoxic drugs might have additive adverse effects.
There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.
Bilberry extract
Anticoagulant/Antiplatelet Drugs
Theoretically, bilberry fruit extract might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro, animal, and clinical research suggest that anthocyanidin extracts from bilberry can inhibit platelet aggregation.
Antidiabetes Drugs
Theoretically, bilberry leaf or fruit extract may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that bilberry leaf extract might have blood glucose-lowering activity. Also, one small clinical trial in patients with type 2 diabetes shows that taking bilberry fruit extract 470 mg as a single dose prior to an oral glucose tolerance test lowers plasma glucose levels when compared with placebo.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Theoretically, bilberry fruit extract might decrease levels of drugs metabolized by CYP2E1.
Animal research shows that exposure to small concentrations of bilberry extract in drinking water for around one month increased CYP2E1 activity by 31%. However, exposure over a 2-month period did not increase CYP2E1 activity. This effect has not been reported in humans.
Erlotinib (Tarceva)
Theoretically, bilberry fruit extract might reduce the efficacy of erlotinib.
In vitro research suggests that bilberry fruit extract and its constituents, delphinidin and delphinidin-3-O-glucoside, inhibit the activity of erlotinib. This interaction has not been reported in humans.
Horse Chestnut seed extract
Anticoagulant/Antiplatelet Drugs
Horse chestnut may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Horse chestnut contains the constituent esculin which has been shown to have antithrombotic effects. Therefore, horse chestnut might have antiplatelet effects. This has not been shown in humans.
