HelloPharmacist Scorecard of Iron Maxx Extreme by Justified Laboratories
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
Iron Maxx Extreme is a 5-ingredient capsule built around arginine compounds. The product contains L-arginine, Arginine Alpha-Ketoglutarate, an L-Arginine Blend, Arginine Ketoisocaproate, Ornithine Alpha-Ketoglutarate, and Glutamine Alpha-Ketoglutarate.
The inactive ingredients are silica, magnesium stearate, and gelatin. These are amino acid derivatives — building blocks the body uses for muscle function, immune support, and circulation.
L-arginine is the most researched ingredient here and the one with documented interactions.
Does it work?
Moderate evidence
The evidence for this product's ingredients is mixed and mostly modest. Ornithine Alpha-Ketoglutarate is rated possibly effective for athletic performance, but evidence is insufficient for dry skin, insomnia, and gum disease.
Glutamine Alpha-Ketoglutarate is rated effective for sickle cell disease and possibly effective for HIV/AIDS-related wasting, postoperative recovery, and critical illness from trauma. No effectiveness ratings are available in our data for L-arginine, Arginine Alpha-Ketoglutarate, or Arginine Ketoisocaproate.
How safe is it?
Well-documented data
L-arginine is generally well tolerated short-term and most commonly causes stomach upset — abdominal pain, bloating, nausea, diarrhea — along with headache, insomnia, and flushing. Long-term safety hasn't been thoroughly studied.
The safety data advises against L-arginine in pregnancy and notes insufficient information for breastfeeding. Ornithine Alpha-Ketoglutarate is generally well tolerated short-term, though long-term safety is unclear.
Safety data advises against it during pregnancy and breastfeeding due to insufficient data. Glutamine Alpha-Ketoglutarate is generally well tolerated in healthy adults but requires medical supervision for those with kidney or liver disease.
People with bipolar disorder should be aware that glutamine has been reported (rarely) to trigger mania or hypomania. The safety data advises caution in breastfeeding.
No pregnancy or breastfeeding data is on file for L-Arginine Blend or Arginine Ketoisocaproate.
Meds to double-check
Moderate interaction found
Before you take Iron Maxx Extreme, double-check these medications with your pharmacist: blood pressure drugs (especially ACE inhibitors and ARBs), potassium-sparing diuretics, blood thinners (anticoagulants and antiplatelet drugs), diabetes medications, and heart drugs like sildenafil or isoproterenol — all Moderate-severity interactions with L-arginine. Also flag anticonvulsants if you take them, due to a Moderate interaction with glutamine.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Iron Maxx Extreme is designed for muscle support and athletic performance, but it's not a simple supplement if you take blood pressure meds, blood thinners, diabetes drugs, or heart medications — L-arginine interacts with all of these. If you're pregnant or breastfeeding, talk to your pharmacist or doctor first.
Anyone with kidney or liver disease should use this only under medical supervision.

Iron Maxx Extreme by Justified Laboratories Drug Interactions
HelloPharmacist Interaction Report
Iron Maxx Extreme by Justified Laboratories contains ingredients with documented interactions affecting a large number of medications.
L-arginine carries Moderate-severity interactions with blood pressure and heart drugs — it can amplify the blood-pressure-lowering effect of antihypertensive medications (including ACE inhibitors and ARBs), potentially causing dangerously low blood pressure. It also raises the risk of high potassium levels (hyperkalemia) when combined with potassium-sparing diuretics or ACE inhibitors.
Read the full breakdown — every affected drug type, severity by severity
L-arginine also has Moderate interactions with blood thinners (anticoagulants and antiplatelet drugs), diabetes medications, and specific heart drugs like sildenafil (Viagra) and isoproterenol, each with the potential for additive effects or low blood pressure. Glutamine Alpha-Ketoglutarate carries a Moderate interaction with anticonvulsant medications, theoretically reducing their effectiveness — though this hasn't been reported in humans yet.
Altogether, these interactions span 449 individual medications. We could not check Arginine Alpha-Ketoglutarate or Arginine Ketoisocaproate.

For each ingredient in Iron Maxx Extreme with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Arginine Blend
Ace Inhibitors (Aceis)
Theoretically, concomitant use of L-arginine and ACE inhibitors may increase the risk for hypotension and hyperkalemia.
Combining L-arginine with some antihypertensive drugs, especially ACE inhibitors, seems to have additive vasodilating and blood pressure-lowering effects. Furthermore, ACE inhibitors can increase potassium levels. Use of L-arginine has been associated with hyperkalemia in some patients. Theoretically, concomitant use of ACE inhibitors with L-arginine may increases the risk of hyperkalemia.
Angiotensin Receptor Blockers (Arbs)
Theoretically, concomitant use of L-arginine and ARBs may increase the risk of hypotension and hyperkalemia.
L-arginine increases nitric oxide, which causes vasodilation. Combining L-arginine with ARBs seems to increase L-arginine-induced vasodilation. Furthermore, ARBs can increase potassium levels. Use of L-arginine has been associated with hyperkalemia in some patients. Theoretically, concomitant use of ARBs with L-arginine may increases the risk of hyperkalemia.
Anticoagulant/Antiplatelet Drugs
Theoretically, concomitant use of L-arginine with anticoagulant and antiplatelet drugs might have additive effects and increase the risk of bleeding.
Preliminary research suggests that L-arginine infusions reduce platelet aggregation in humans. The clinical significance of this effect is unclear.
Antidiabetes Drugs
Theoretically, concomitant use of L-arginine might have additive effects with antidiabetes drugs.
Preliminary clinical research shows that L-arginine decreases blood glucose levels in patients with type 2 diabetes.
Antihypertensive Drugs
Theoretically, concomitant use of L-arginine and antihypertensive drugs may increase the risk of hypotension.
L-arginine increases nitric oxide, which causes vasodilation. Clinical evidence shows that L-arginine can reduce blood pressure in some individuals with hypertension. Furthermore, combining L-arginine with some antihypertensive drugs seems to have additive vasodilating and blood pressure-lowering effects.
Isoproterenol (Isuprel)
Theoretically, concurrent use of isoproterenol and L-arginine might result in additive effects and hypotension.
Preliminary clinical evidence suggests that L-arginine enhances isoproterenol-induced vasodilation in patients with essential hypertension or a family history of essential hypertension.
Potassium-Sparing Diuretics
Theoretically concomitant use of potassium-sparing diuretics with L-arginine may increases the risk of hyperkalemia.
Potassium-sparing diuretics can increase potassium levels. Use of L-arginine has been associated with hyperkalemia in some patients.
Sildenafil (Viagra)
Theoretically, concurrent use of sildenafil and L-arginine might increase the risk for hypotension.
In vivo, concurrent use of L-arginine and sildenafil has resulted in increased vasodilation. Theoretically, concurrent use might have additive vasodilatory and hypotensive effects. However, in studies evaluating the combined use of L-arginine and sildenafil for erectile dysfunction, hypotension was not reported.
Testosterone
Theoretically, concomitant use of L-arginine and testosterone might have additive effects.
In clinical research, L-arginine increases the level of testosterone in male patients with erectile dysfunction. The clinical significance of this finding is unclear.
