L-arginine and Aliskiren

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Aliskiren

Aliskiren falls under ANTIHYPERTENSIVE DRUGS — the group L-arginine interacts with.

Theoretically, concomitant use of L-arginine and antihypertensive drugs may increase the risk of hypotension.
L-arginine increases nitric oxide, which causes vasodilation. Clinical evidence shows that L-arginine can reduce blood pressure in some individuals with hypertension. Furthermore, combining L-arginine with some antihypertensive drugs seems to have additive vasodilating and blood pressure-lowering effects.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know if you're pairing L-arginine with Tekturna (aliskiren): both can nudge your blood pressure in the same direction, downward. Aliskiren works by calming the renin system so your vessels relax, while L-arginine boosts nitric oxide, which also widens vessels. Stack those two effects and your pressure could dip a little more than expected.

If that happens, you might feel lightheaded, dizzy, tired, or a bit faint when you stand up too fast. To be fair, this concern is mostly theoretical and based on how these two work rather than strong studies, so for many people it's mild. Still, if you're already on aliskiren, please check with your pharmacist or doctor before adding L-arginine, and keep an eye on your home blood pressure readings.

From our pharmacist’s overview on the Aliskiren page

“Aliskiren (Tekturna) has one job, keeping blood pressure down, and most of this list pulls on that same rope. Of the 227 supplements with an interaction on file, 119 have blood-pressure effects of their own, stacking with the drug until pressure drops too far (fish oil is a common one) or pushing back against it.”

About L-arginine

What is L-arginine?

L-arginine
  • High blood pressure
  • Erectile dysfunction
  • Athletic performance
  • Chest pain (angina)
  • Improving blood flow

L-arginine is an amino acid that the body uses to make nitric oxide, a substance that helps blood vessels relax and widen. It is popularly used for blood pressure, erectile dysfunction, and exercise, but the evidence is mixed and stronger for some uses than others. It is generally well tolerated in healthy adults, but talk with a pharmacist or doctor before using it, especially if you have heart problems or take other medicines.

About Aliskiren

What Is Aliskiren Used For?

Brand name: Tekturna

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.

Managing blood pressure is usually part of a broader plan that may also include diet changes, exercise, cholesterol control, and other medications. Many people need more than one blood pressure drug to reach their goal.
