What’s inside
Full disclosure
This product contains one active ingredient: L-carnitine free form at 500 mg per serving. L-carnitine is an amino acid your body uses to transport fatty acids into cells so they can be burned for energy.
The inactive ingredients are vegetable cellulose, food glaze, stearic acid, silica, and less than 2% of other additives — these are fillers and capsule materials with no therapeutic role.
Does it work?
Strong evidence
L-carnitine is effective for treating L-carnitine deficiency. It's possibly effective for high triglycerides (hyperlipidemia), congestive heart failure (a weakened heart that can't pump well), and angina (chest pain from reduced blood flow to the heart).
The evidence isn't strong enough to call it proven for those conditions, but the data we hold suggests a benefit is plausible.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at normal doses, but high doses can upset your stomach and cause a fishy body odor. The most common side effects are abdominal cramps, pain, diarrhea, heartburn, nausea, reduced appetite, and vomiting.
Seizures are rare but have been reported. There isn't enough reliable safety data to recommend this supplement on its own without medical supervision — talk to your doctor or pharmacist first.
For breastfeeding, the safety data is limited, so caution is advised if you're nursing.
Meds to double-check
Moderate interaction found
Before taking this L-carnitine, check with your doctor or pharmacist if you use any blood thinners, especially warfarin (Coumadin) or acenocoumarol (Sintrom) — L-carnitine may increase their effects and raise your bleeding risk. Also mention it if you take thyroid hormone replacement, as L-carnitine might reduce how well it works.
The bottom line
Scorecard at a glanceFully disclosed formula with strong clinical evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
L-carnitine may help if you have a documented deficiency or are interested in its possibly effective uses for heart and cholesterol issues, but it's not something to start without guidance. If you take blood thinners or thyroid medication, you'll need to clear it with your doctor or pharmacist first — the interactions are real and could affect how well your medicines work.

L-Carnitine 500 mg by Vitamin World Drug Interactions
HelloPharmacist Interaction Report
Vitamin World's L-Carnitine 500 mg interacts with blood thinners and thyroid hormones.
The most serious concern is with acenocoumarol (Sintrom), a blood thinner similar to warfarin — L-carnitine may boost its anticoagulant effects, and there are at least two case reports of elevated INR (a measure of how thin your blood is) when used together. Warfarin (Coumadin) may also be affected in the same way, though the interaction is less certain.
Read the full breakdown — every affected drug type, severity by severity
L-carnitine might also reduce how well thyroid hormone replacement works by blocking thyroid hormone from entering your cells, which could be a problem if you're taking it for an underactive thyroid (hypothyroidism).
Altogether, these interactions span 19 individual medications. If you take warfarin, acenocoumarol, or any thyroid medication, run them through the checker below or talk with your doctor or pharmacist before starting this supplement.

For each ingredient in L-Carnitine 500 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine Free Form
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
