The capsule also holds inactive ingredients — hydroxypropyl methylcellulose, vegetable magnesium stearate, silicon dioxide, and microcrystalline cellulose — which are fillers and binders that hold the capsule together.
Does it work?
Moderate evidence
The evidence for L-citrulline is mixed. It's rated possibly effective for athletic performance.
For sarcopenia (age-related muscle loss), it's rated possibly ineffective. The data we hold shows insufficient reliable evidence to rate it for heart failure, cardiovascular disease, Reye syndrome, or diabetes, so we can't say whether it works for those conditions.
How safe is it?
Well-documented data
L-citrulline is generally well tolerated in healthy adults, though long-term safety hasn't been fully studied. The most common side effects reported are gastrointestinal discomfort, heartburn, and stomach upset.
A small number of people with pulmonary hypertension reported cough while taking it. There isn't enough safety data during pregnancy, so the facts advise against use.
Safety while breastfeeding is unknown, so supplemental use is not advised during that time either. If you're pregnant, planning to become pregnant, or breastfeeding, talk with your doctor or pharmacist about whether this product is right for you.

With blood pressure medications, L-citrulline may have an additive effect. It's converted in your body to L-arginine, which increases nitric oxide and relaxes blood vessel walls.
Combined with drugs that already lower blood pressure, this could theoretically increase the risk of low blood pressure (hypotension). However, a meta-analysis of 5 small studies found that taking 3–6 grams daily for [note: the effect text is incomplete in the data].
With PDE-5 inhibitors like sildenafil (Viagra) or tadalafil (Cialis), the concern is similar: both work through vasodilation pathways, so taking them together might amplify the drop in blood pressure.

L-Citrulline
Antihypertensive Drugs
Theoretically, concomitant use of L-citrulline with antihypertensive drugs might have additive effects and increase the chance of hypotension.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. However, a meta-analysis of 5 small clinical studies suggests that taking L-citrulline 3-6 grams daily for 1-8 weeks does not lower blood pressure when compared with control.
Phosphodiesterase-5 Inhibitors
Theoretically, concurrent use of phosphodiesterase-5 (PDE-5) inhibitors and L-citrulline might result in additive vasodilation.
L-citrulline is converted to L-arginine, which can increase nitric oxide and cause vasodilation. Theoretically, taking L-arginine with PDE-5 inhibitors might have additive vasodilatory and hypotensive effects. However, in studies evaluating the combined use of L-arginine and sildenafil for erectile dysfunction, hypotension was not reported.
