How safe is it?
Well-documented data
L-theanine is generally well tolerated in healthy adults for short-term use, though long-term safety isn't well studied. Side effects can include headaches, drowsiness, longer sleep, and more vivid dreams.
There's one old report of facial tics in a child, but the tics went away when the supplement stopped, and the child had a history of tics anyway, so it's not clear theanine caused it. For pregnancy and breastfeeding, there isn't enough safety data, so it's best to avoid it during both.

The main one is blood pressure medications (antihypertensive drugs). Theanine can lower blood pressure on its own, and animal studies suggest it might strengthen the effect of your BP meds.
If you're on a blood pressure drug, that's worth a conversation with your doctor or pharmacist before you add this in.
Second, there's a theoretical concern with CNS depressants — drugs like sleep aids or anti-anxiety medications. Theanine might pile on the drowsiness, though we're not sure how much this matters in real life.
And while some studies hint theanine could affect serotonin levels, no one's actually reported serotonin syndrome from it, so that risk is pretty low.

L-Theanine
Antihypertensive Drugs
Theanine might lower blood pressure, potentiating the effects of antihypertensive drugs.
Animal research shows that theanine can lower blood pressure in spontaneously hypertensive animals. Theoretically, concomitant use of theanine and antihypertensive drugs might potentiate the antihypertensive activity.
Cns Depressants
Theoretically, theanine might have additive sedative effects when used in conjunction with CNS depressants. However, it is unclear if this concern is clinically relevant.
Theoretically, theanine may compete with glutamate and/or increase plasma gamma-aminobutyric acid (GABA) levels, which could cause CNS depression. In one clinical study, some subjects taking oral theanine reported drowsiness.
Serotonergic Drugs
Clinical studies regarding the effects of L-theanine on serotonin levels are conflicting. Some studies suggest it can increase serotonin levels in the brain while others report that it may decrease them. Nevertheless, there have been no reports of l-theanine being a causative agent in serotonergic-related side effects or serotonin syndrome.
