Clinical studies regarding the effects of L-theanine on serotonin levels are conflicting. Some studies suggest it can increase serotonin levels in the brain while others report that it may decrease them. This combination cold product mixes a pain/fever reducer (acetaminophen), an antihistamine (chlorpheniramine), and a decongestant (phenylephrine). The only theoretical concern here ties to serotonin, since chlorpheniramine has mild serotonin activity and theanine may nudge serotonin levels in the brain. Honestly though, the research on theanine and serotonin is all over the map, with some studies showing a rise and others a drop, and there are no real reports of it causing problems.

So in everyday terms, this is a very low-level, mostly-on-paper concern. I wouldn't expect issues like agitation or a racing heart from pairing them. Just be mindful of the usual effects from this cold medicine itself, such as drowsiness, dry mouth, or feeling jittery from the decongestant. If you take other serotonin-affecting medicines, or you simply want peace of mind, run it by your pharmacist or doctor first.
