Theanine and Phenytoin

Here is the interaction we found between these two, with the evidence behind it.

Minor

CNS DEPRESSANTS

Why this applies to Phenytoin

Phenytoin falls under CNS DEPRESSANTS — the group Theanine interacts with.

Theoretically, theanine might have additive sedative effects when used in conjunction with CNS depressants. However, it is unclear if this concern is clinically relevant.
Theoretically, theanine may compete with glutamate and/or increase plasma gamma-aminobutyric acid (GABA) levels, which could cause CNS depression. In one clinical study, some subjects taking oral theanine reported drowsiness.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for phenytoin (Dilantin) and theanine. This pairing is grouped together because phenytoin works in the brain and theanine may have a mild calming, slightly sedating effect. In theory, stacking the two could add up to a bit more drowsiness, tiredness, or feeling mentally foggy. This isn't about theanine changing your phenytoin levels, it's just two things that can each make you sleepy.

I want to keep this in perspective. The concern is theoretical and considered unlikely to cause real trouble. Only some people in one study even noticed drowsiness from theanine. Still, since phenytoin already can cause dizziness or grogginess, it's worth a quick chat with your pharmacist or doctor before adding theanine, especially before driving.

From our pharmacist’s overview on the Phenytoin page

“Phenytoin (Dilantin) is a seizure medicine with a narrow therapeutic index, meaning small changes in its blood level can bring seizures back or tip over into toxicity, so I treat its supplement list with real seriousness. About Theanine

What is Theanine?

Theanine
  • Anxiety and stress
  • Relaxation and calm
  • Sleep support
  • Focus and cognitive function

Theanine (usually L-theanine) is an amino acid found naturally in tea leaves that many people take to feel calmer and less stressed without strong drowsiness. Early research suggests it may help with relaxation and stress, but evidence for most uses is still limited. It appears generally well tolerated for short-term use, but check with your pharmacist or doctor before starting it.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
