Theanine and Levetiracetam

Here is the interaction we found between these two, with the evidence behind it.

Minor

CNS DEPRESSANTS

Why this applies to Levetiracetam

Levetiracetam falls under CNS DEPRESSANTS — the group Theanine interacts with.

Theoretically, theanine might have additive sedative effects when used in conjunction with CNS depressants. However, it is unclear if this concern is clinically relevant.
Theoretically, theanine may compete with glutamate and/or increase plasma gamma-aminobutyric acid (GABA) levels, which could cause CNS depression. In one clinical study, some subjects taking oral theanine reported drowsiness.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with levetiracetam (Keppra) and theanine: this isn't about one changing the other's blood levels. Levetiracetam isn't a prodrug, and theanine doesn't really speed up or slow down how your body clears it. The concern is simpler. Theanine may nudge calming brain chemicals like GABA, and levetiracetam can already cause drowsiness, tiredness, or a foggy, sluggish feeling. Stacked together, those calm-down effects could in theory add up a bit.

Honestly, this is a theoretical, minor concern based on how each works, not strong real-world data, so I wouldn't lose sleep over it. Still, if you notice extra drowsiness, just loop in your pharmacist or doctor before changing anything.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Levetiracetam page

“I would read this page carefully rather than casually. Levetiracetam (Keppra, Spritam) controls seizures, and both hazards on this list run through that fact.”

About Theanine

What is Theanine?

Theanine
  • Anxiety and stress
  • Relaxation and calm
  • Sleep support
  • Focus and cognitive function

Theanine (usually L-theanine) is an amino acid found naturally in tea leaves that many people take to feel calmer and less stressed without strong drowsiness. Early research suggests it may help with relaxation and stress, but evidence for most uses is still limited. It appears generally well tolerated for short-term use, but check with your pharmacist or doctor before starting it.

About Levetiracetam

What Is Levetiracetam Used For?

Brand names: Keppra, Elepsia XR, Spritam

Levetiracetam is used to treat several types of seizures. It treats partial-onset seizures (seizures that start in one area of the brain) and is used as add-on therapy for myoclonic seizures (sudden, brief muscle jerks) in people with juvenile myoclonic epilepsy aged 12 and older, and for primary generalized tonic-clonic seizures (grand mal seizures) in people with idiopathic generalized epilepsy aged 6 and older.

Age eligibility varies by product. Keppra oral and Roweepra tablets treat partial-onset seizures in patients as young as 1 month. Spritam and levetiracetam tablets for oral suspension treat partial-onset seizures in patients 4 years and older weighing more than 20 kg. Extended-release products — including Elepsia XR, Keppra XR, and Levetiracetam ER — are approved only for partial-onset seizures in patients 12 years and older. Injectable forms are used intravenously when a patient temporarily cannot take oral medication.
