Here's the simple version: both GHB and levetiracetam (Keppra) calm down the brain and nervous system. When you stack two things that slow the brain at the same time, those calming effects can pile on top of each other. With levetiracetam, that means the drowsiness, dizziness, and fatigue it can already cause may become much stronger, and GHB can push that all the way to dangerously slow or stopped breathing.

This isn't about how your liver breaks the drug down, it's about overlapping sedation. Levetiracetam isn't a prodrug, so the concern is simply added-up CNS depression. Honestly, GHB outside of a prescribed setting isn't safe to take with anything. Please don't change your seizure medicine on your own, and talk with your doctor or pharmacist about any GHB use.

Theoretically, GHB may reduce the effects of anticonvulsant medications.
GHB has been reported to cause seizures, and may theoretically reduce the effectiveness of anticonvulsants. Also, concomitant use of GHB with sedative anticonvulsants may increase the risk of serious CNS and respiratory depression.

First, a real safety note: GHB sold outside of a prescription isn't a supplement at all. It's a powerful nervous-system depressant and a controlled substance, and on its own it can cause coma and stopped breathing. Mixing it with anything is risky. With levetiracetam (Keppra), there are two concerns worth knowing about.

The first is that GHB has actually been reported to trigger seizures, so it could theoretically work against what Keppra is trying to do and make a breakthrough seizure more likely. The second is overlap in side effects: both can cause drowsiness, dizziness, and slowed thinking, so together you may feel more sedated than expected. To be clear, this is based on weak, anecdotal evidence, not solid studies. Levetiracetam isn't a prodrug, so this isn't about activation. Please talk with your doctor or pharmacist before combining them.

What is Gamma-hydroxybutyrate (ghb)?

GHB (gamma-hydroxybutyrate) is a powerful central nervous system depressant that is NOT a safe dietary supplement. In the U.S. it is a controlled substance, and only a tightly regulated prescription form (sodium oxybate) is legal for narcolepsy. Non-prescription GHB can cause coma, breathing failure, and death, and it is also a known 'date-rape' drug — it should not be taken outside of medical supervision.

About Levetiracetam

What Is Levetiracetam Used For?

Brand names: Keppra, Elepsia XR, Spritam

Levetiracetam is used to treat several types of seizures. It treats partial-onset seizures (seizures that start in one area of the brain) and is used as add-on therapy for myoclonic seizures (sudden, brief muscle jerks) in people with juvenile myoclonic epilepsy aged 12 and older, and for primary generalized tonic-clonic seizures (grand mal seizures) in people with idiopathic generalized epilepsy aged 6 and older.

Age eligibility varies by product. Keppra oral and Roweepra tablets treat partial-onset seizures in patients as young as 1 month. Spritam and levetiracetam tablets for oral suspension treat partial-onset seizures in patients 4 years and older weighing more than 20 kg. Extended-release products — including Elepsia XR, Keppra XR, and Levetiracetam ER — are approved only for partial-onset seizures in patients 12 years and older. Injectable forms are used intravenously when a patient temporarily cannot take oral medication.
