Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: lemon balm aerial parts extract (the leaves and stems). The liquid is made with glycerin and distilled water as inactive ingredients.
Does it work?
Moderate evidence
The evidence we hold shows lemon balm is possibly effective for cold sores (herpes labialis), stress, and depression. That means some clinical research supports its use for these, but more and larger trials would strengthen the case.
If you're thinking about it for one of these reasons, talk it over with your doctor or pharmacist to see if it fits your situation.
How safe is it?
Well-documented data
Lemon balm is generally well tolerated for short-term use in healthy adults, though we don't have solid long-term safety data at high doses. In clinical trials, reported side effects like dizziness and sedation didn't show up more often than placebo, and combining it with alcohol didn't affect reaction time or thinking.
One case report described heart rhythm changes (atrioventricular block and QT prolongation) in a woman taking a multi-ingredient supplement containing lemon balm, but it's unclear whether lemon balm alone caused it — symptoms resolved when she stopped the product. During pregnancy, there isn't enough reliable safety data, so it's best avoided unless your doctor says otherwise.
While breastfeeding, safety information is limited — check with your doctor or pharmacist before using.
Meds to double-check
Moderate interaction found
Before you take this, double-check your medications against CNS depressants (sedatives, anti-anxiety drugs, sleep aids) and thyroid hormone replacement therapy. Both categories have documented moderate-severity interactions with lemon balm.
Use the search tool on this page to confirm your exact medications.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Lemon balm may help with cold sores, stress, or low mood, and it's usually well tolerated in the short term. If you take a CNS depressant (like a sedative or anxiety medication) or thyroid replacement hormone, you'll want to check with your doctor or pharmacist before starting — this product can interact with both.
Talk it over with your own healthcare provider before adding it to your routine.

Lemon Balm Alcohol-Free by Herb Pharm Drug Interactions
HelloPharmacist Interaction Report
Herb Pharm's Lemon Balm Alcohol-Free has documented interactions with two categories of medication.
The most serious concern is with CNS depressants — drugs that slow your central nervous system, like sedatives, sleeping pills, and some anti-anxiety medications. Lemon balm itself appears to have mild CNS depressant activity, so using them together could add up to stronger drowsiness or sedation than either one alone.
Read the full breakdown — every affected drug type, severity by severity
Lemon balm may also interfere with thyroid hormone replacement therapy. The herb's compounds can bind to thyroid-stimulating hormone (TSH) in the lab, and animal studies show it can lower thyroid hormone levels.
If you take levothyroxine or another thyroid replacement, this matters.
Additionally, lemon balm has been checked for interactions across 264 individual medications. Even so, always run your exact prescriptions through the search tool on this page before you start, especially if you're on a CNS depressant or thyroid medication.

Lemon Balm Aerial Parts Extract
Cns Depressants
Theoretically, concomitant use of lemon balm might have additive effects with CNS depressant drugs.
Lemon balm seems to have CNS depressant activity in animals and in humans.
Thyroid Hormone
Theoretically, lemon balm might interfere with thyroid hormone replacement therapy.
In vitro, constituents of lemon balm extract bind to thyroid stimulating hormone (TSH), preventing TSH receptor-binding and leading to the inhibition of TSH-stimulated adenylate cyclase activity. In animals, lemon balm extract has been shown to decrease levels of circulating TSH and inhibit thyroid secretion.
