Lemon balm is generally well tolerated for short-term use in most healthy adults, but high doses and long-term safety are not well studied. Oral use has been reported to cause dizziness and sedation, though these effects did not occur more often than with placebo in research.

Topical 1% cream used on cold sores has been linked to a few cases of irritation, again not more often than placebo. One case report describes a 25-year-old woman with a heart rhythm disturbance after using a different multi-ingredient product that contained lemon balm.

It is unclear whether lemon balm caused it. During pregnancy, our safety notes advise avoiding it unless your doctor advises otherwise, because reliable safety data is limited.

While breastfeeding, safety information is also limited, so use caution and talk with your doctor first.

Theoretically, concomitant use of lemon balm might have additive effects with CNS depressant drugs.

Theoretically, lemon balm might interfere with thyroid hormone replacement therapy.
In vitro, constituents of lemon balm extract bind to thyroid stimulating hormone (TSH), preventing TSH receptor-binding and leading to the inhibition of TSH-stimulated adenylate cyclase activity. In animals, lemon balm extract has been shown to decrease levels of circulating TSH and inhibit thyroid secretion.
