Lemon balm is rated possibly effective for cold sores (herpes labialis) and for stress and mood support, based on the clinical evidence we hold. That means some trials show promise, but the data isn't yet strong enough to say it definitely works for these purposes.

How safe is it?
Well-documented data
Lemon balm is generally well tolerated for short-term use in healthy adults, though long-term safety and high-dose effects haven't been thoroughly studied. Some trial participants reported dizziness or drowsiness, but these didn't occur more often than placebo.
One case of cardiac rhythm changes (heart block and QT prolongation) was reported in a young woman using a multi-ingredient product containing lemon balm, but it's unclear whether lemon balm alone was responsible; her symptoms resolved within days of stopping the product. During pregnancy, there isn't enough reliable safety data, so it's best avoided unless your doctor advises otherwise.
While breastfeeding, safety information is limited — talk with your doctor or pharmacist before use. Topical use (1% cream on cold sores) has occasionally caused local irritation, but again not more often than placebo.

In vitro, constituents of lemon balm extract bind to thyroid stimulating hormone (TSH), preventing TSH receptor-binding and leading to the inhibition of TSH-stimulated adenylate cyclase activity. In animals, lemon balm extract has been shown to decrease levels of circulating TSH and inhibit thyroid secretion.
