Lemon balm is generally well tolerated for short-term use in most healthy adults, but high doses and long-term safety are not well studied. Oral use has been reported to cause dizziness and sedation, though these do not appear to occur more often than with placebo.
Research also found that using lemon balm with alcohol did not affect reaction time or cognitive performance. A case report describes a 25-year-old woman who developed a heart rhythm problem after using a different multi-ingredient product containing lemon balm.
It is unclear whether lemon balm caused it. For pregnancy, our safety data advises avoiding lemon balm unless your doctor advises otherwise, because there is not enough reliable safety data.
For breastfeeding, the data is limited, so use caution and talk with your doctor first.

Theoretically, concomitant use of lemon balm might have additive effects with CNS depressant drugs.
Lemon balm seems to have CNS depressant activity in animals and in humans.
Thyroid Hormone
Theoretically, lemon balm might interfere with thyroid hormone replacement therapy.
In vitro, constituents of lemon balm extract bind to thyroid stimulating hormone (TSH), preventing TSH receptor-binding and leading to the inhibition of TSH-stimulated adenylate cyclase activity. In animals, lemon balm extract has been shown to decrease levels of circulating TSH and inhibit thyroid secretion.
