Moderate evidence
Malic acid is possibly effective for dry mouth. For acne, gastroesophageal reflux disease (GERD), fatigue, fibromyalgia, and psoriasis, the evidence isn't established enough to rate it — we don't have reliable data showing whether it works for these uses.
How safe is it?
Well-documented data
Malic acid taken by mouth appears to be well tolerated at typical food amounts, though supplement safety at higher doses hasn't been well studied. The safety notes advise caution with supplement doses and recommend talking to your doctor before using it.
Topically, it's well tolerated on mouth tissues, but it can cause eye irritation, skin irritation, and peeling on skin — in some cases moderate to strong irritation. In one study, people who avoided malic and citric acid foods then ate high amounts experienced hives and delayed skin reactions.
Diarrhea and nausea have been reported with a combination product containing malic acid and magnesium, though it's unclear which ingredient caused it since magnesium itself has a laxative effect at that dose.
