The research shows L-glutamine is effective for sickle cell disease. It's rated possibly effective for HIV/AIDS-related wasting, recovery after surgery, and critical illness from trauma.

How safe is it?
Well-documented data
L-glutamine is generally well tolerated when taken by mouth in healthy adults. The most common side effects are gastrointestinal—belching, bloating, constipation, diarrhea, flatulence, nausea, vomiting, and stomach pain.
Headache and musculoskeletal pain have also been reported. A couple of cases of mood changes (mania and hypomania) occurred in people with bipolar disorder taking glutamine, though they're rare.
If you have kidney or liver disease, use this only under medical supervision. There isn't enough safety data to recommend supplemental doses during pregnancy or breastfeeding unless your doctor advises it—talk with your healthcare provider for personalized guidance.
