Lactobacillus acidophilus is generally well tolerated in healthy adults and children, though it's considered low risk enough that you should check with your doctor before starting it. In rare cases, the live bacteria have been linked to serious infections like bacteremia, sepsis, or endocarditis, mostly in people whose immune systems are compromised or who have underlying conditions — translocation of bacteria already present in the body.

More commonly, taking L. acidophilus with other probiotics may cause mild gastrointestinal side effects like bloating, flatulence, abdominal pain, or diarrhea, though these are uncommon. Inulin is well tolerated in food amounts but often causes gas, bloating, diarrhea, constipation, and stomach cramps, especially at higher doses (over 30 grams).

Anaphylaxis is rare but has occurred. Data on safety at supplement doses is limited, so talk to your doctor before using concentrated inulin products.

During pregnancy, L. acidophilus is possibly safe, and inulin is likely safe — but data for the other three bacterial strains during pregnancy and breastfeeding aren't on file.

L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
