Lactobacillus acidophilus is generally well tolerated in healthy adults and children, though you should check with your doctor before starting. Uncommon gastrointestinal side effects — such as stomach discomfort, abdominal pain, indigestion, gas, bloating, diarrhea, vomiting, and burping — have been reported when L. acidophilus is combined with other probiotics.

In rare cases, L. acidophilus organisms have been found in serious infections like sepsis or meningitis, though this typically happens in people whose bacteria have translocated from other parts of the body. Bifidobacterium bifidum is generally well tolerated in healthy people, but those with weakened immune systems should be cautious.

Bloating and gas can occur with probiotic use, though these haven't been specifically reported from B. bifidum alone. A single case of vomiting and fever was reported in a child taking B. bifidum with L. acidophilus, though it's unclear whether the probiotics caused it.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
