Rare reports of bleeding — including one case of brain hemorrhage — have been linked to nattokinase, especially when combined with aspirin or other blood thinners.

Hypersensitivity reactions, including severe throat swelling and anaphylaxis, have also been reported in people sensitive to natto or its components. There isn't enough safety data to use nattokinase during pregnancy, so it's best avoided.

Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
