The evidence for gotu kola is mixed. It's possibly effective for venous insufficiency (poor circulation in the veins) and for burns.
For radiation dermatitis (skin damage from cancer treatment), cognitive function, and anxiety, the evidence is either insufficient or points toward it being ineffective. For Alzheimer's disease, we simply don't have enough reliable data to rate it either way.
How safe is it?
Well-documented data
Gotu kola is generally well tolerated short-term, though quality evidence is limited. The most common side effects when taken by mouth are stomach irritation and nausea.
Topically (on skin), it can occasionally cause eczema or allergic contact dermatitis with redness, itching, and burning. The main safety concern is liver toxicity — rare, but documented in a small number of case reports.
One clinical trial found no liver damage at 120 mg daily for 6 months, yet caution is warranted if you have liver disease or take multiple drugs that stress the liver. Gotu kola is traditionally avoided in pregnancy because safety data is limited.
It should not be used during breastfeeding, as safety is unknown.

Cns Depressants
Theoretically, taking gotu kola might increase the sedative effects of CNS depressants.
In vitro research suggests that gotu kola may have sedative effects via binding of GABA receptors.
Hepatotoxic Drugs
Theoretically, taking gotu kola with hepatotoxic drugs might have additive adverse effects.
There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.
