Andrographis is generally well tolerated over the short term, but long-term safety and product quality aren't well established. At higher doses — particularly 5–10 mg/kg of the active compound andrographolide — or beyond 14 days, side effects become more likely.

Common ones include abdominal discomfort, altered taste, diarrhea, dizziness, fatigue, headache, nausea, vomiting, itching, and rash. Serious allergic reactions including anaphylaxis are rare but can occur.

Skin reactions — ranging from rash and hives to, in rare cases, Stevens-Johnson syndrome (a severe blistering condition) — have been reported. Avoid this product during pregnancy because of theoretical and animal-study concerns about reproductive effects.

There isn't enough safety information to use it while breastfeeding, so skip it if you're nursing.

Animal research shows that andrographis extract, or the constituent andrographolide, taken orally with etoricoxib decreases the bioavailability of etoricoxib. However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
