Short-term, tribulus is generally well tolerated in healthy adults, but long-term safety data are limited and quality varies between products. Avoid it during pregnancy—it may affect hormones and safety isn't established.

Also avoid it while breastfeeding due to lack of reliable safety information. Rare serious side effects have been reported: liver and kidney injury, seizures, and a painful erection with impaired sexual function.

More common are stomach upset—abdominal pain, cramping, nausea, vomiting, diarrhea, or constipation—though one study found these occurred at similar rates in people taking tribulus and those on placebo. One case report described someone who drank 2 liters of tribulus water daily for two days and developed weakness, seizures, liver inflammation, and acute kidney injury; they improved after the supplement was stopped and they had dialysis.
