The evidence picture is mixed. Strontium is rated effective for dental sensitivity and bone metastases (cancer-related bone pain), and likely effective for osteoporosis.
It's possibly effective for osteoarthritis and prostate cancer. Bioperine's effectiveness for hay fever, asthma, athletic performance, headache, depression, and diarrhea all fall into insufficient reliable evidence — meaning the research doesn't yet support or rule out benefit.

How safe is it?

Strontium carries important cautions. It's linked to serious heart and clotting risks, especially the ranelate form — clinical studies show it raises the risk of blood clots (venous thromboembolism) by about 50% and heart attack by roughly 1.6-fold in people with osteoporosis.
Common side effects include diarrhea, headache, nausea, and skin reactions like dermatitis. Strontium should be avoided in pregnancy (not enough safety data) and while breastfeeding.
Bioperine from black pepper is generally well tolerated at food amounts, but concentrated supplements warrant caution — the main side effects are burning aftertaste, indigestion, and reduced taste perception. Rarely, allergic reactions occur.

OsteoV SC is an all-natural strontium citrate supplement designed to be combined with the plant calcium, minerals and bone supporting nutrients found in NuMedica's osteo Vegan advanced bone support formula. Strontium, a trace mineral found in seawater and soil, is a mineral with similar properties to calcium. Research has shown that strontium may play a role in the formation of osteoblasts, new bone forming cells, while at the same time slowing the breakdown of old bone by inhibiting osteoclasts. OsteoV SC by NuMedica contains Bioperine (from black pepper) and strontium, both of which interact with medications.
Bioperine's most serious interactions are Moderate-severity effects on several drug classes. It can increase blood levels of heart medications like propranolol, potentially raising side effects — black pepper slows how your body clears this drug and increases how much you absorb.
Strontium carries Moderate interactions with estrogens, antacids, and antibiotics. Estrogens may reduce how much strontium your body excretes, theoretically raising its effects.
Antacids significantly reduce strontium absorption — calcium can cut it by 60% to 70%, so space them 2 hours apart. Quinolone and tetracycline antibiotics may bind with strontium in your stomach, lowering antibiotic absorption; again, separate dosing by at least 2 hours.
Bioperine also interacts with the HIV medication nevirapine, the tuberculosis drug rifampin, the seizure medication phenytoin, the transplant drug cyclosporine, the sedative pentobarbital, the cholesterol drug atorvastatin, and theophylline (a breathing medication) — all Moderate severity, all via increased blood levels or prolonged effect. Altogether, these interactions span 1,046 individual medications.

Bioperine
Anticoagulant/Antiplatelet Drugs
Theoretically, black pepper might increase the risk of bleeding when taken with antiplatelet or anticoagulant drugs.
In vitro research shows that piperine, a constituent of black pepper, seems to inhibit platelet aggregation. This has not been reported in humans.
Antidiabetes Drugs
Theoretically, black pepper might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research shows that piperine, a constituent of black pepper, can reduce blood glucose levels. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Atorvastatin (Lipitor)
Theoretically, black pepper might increase blood levels of atorvastatin.
Animal research shows that taking piperine, a constituent of black pepper, 35 mg/kg can increase the maximum serum concentration of atorvastatin three-fold. This has not been reported in humans.
Cyclosporine (Neoral, Sandimmune)
Theoretically, black pepper might increase the effects and side effects of cyclosporine.
In vitro research shows that piperine, a constituent of black pepper, increases the bioavailability of cyclosporine. This has not been reported in humans.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, black pepper might increase levels of drugs metabolized by CYP2D6.
In vitro research suggests that some constituents of black pepper inhibit CYP2D6. This has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, black pepper might increase levels of drugs metabolized by CYP3A4.
In vitro research and pharmacokinetic simulation data suggest that piperine, a constituent of black pepper, as well as the pepper fruit seem to inhibit CYP3A4. This has not been reported in humans.
Lithium
Theoretically, black pepper might increase blood levels of lithium due to its diuretic effects. The dose of lithium might need to be reduced.
Black pepper is thought to have diuretic properties.
Nevirapine (Viramune)
Black pepper might increase blood levels of nevirapine.
Clinical research shows that piperine, a constituent of black pepper, increases the plasma concentration of nevirapine. However, no adverse effects were observed in this study.
P-Glycoprotein Substrates
Theoretically, black pepper might increase levels of P-glycoprotein substrates.
In vitro research shows that piperine, a constituent of black pepper, seems to inhibit P-glycoprotein.
Pentobarbital (Nembutal)
Theoretically, black pepper might increase the sedative effects of pentobarbital.
Animal research shows that piperine, a constituent of black pepper, increases pentobarbital-induced sleeping time.
Phenytoin (Dilantin)
Black pepper might increase blood levels of phenytoin.
Clinical research shows that piperine, a constituent of black pepper, seems to increase absorption, slow elimination, and increase levels of phenytoin. Taking a single dose of black pepper 1 gram along with phenytoin seems to double the serum concentration of phenytoin. Consuming a soup with black pepper providing piperine 44 mg/200 mL of soup along with phenytoin also seems to increase phenytoin levels when compared with consuming the same soup without black pepper.
Propranolol (Inderal)
Black pepper might increase blood levels of propranolol.
Clinical research shows that piperine, a constituent of black pepper, seems to increase absorption and slow elimination of propranolol.
Rifampin (Rifadin)
Black pepper might increase blood levels of rifampin.
Clinical research shows that piperine, a constituent of black pepper, seems to increase absorption and serum levels of rifampin.
Theophylline
Black pepper might increase blood levels of theophylline.
Clinical research shows that piperine, a constituent of black pepper, seems to increase absorption and slow elimination of theophylline.
Amoxicillin (Amoxil, Trimox)
Theoretically, black pepper might increase the effects and side effects of amoxicillin.
Animal research shows that taking piperine, a constituent of black pepper, with amoxicillin increases plasma levels of amoxicillin. This has not been reported in humans.
Carbamazepine (Tegretol)
Theoretically, black pepper might increase blood levels of carbamazepine, potentially increasing the effects and side effects of carbamazepine.
One clinical study in patients taking carbamazepine 300 mg or 500 mg twice daily shows that taking a single 20 mg dose of purified piperine, a constituent of black pepper, increases carbamazepine levels. Piperine may increase carbamazepine absorption by increasing blood flow to the GI tract, increasing the surface area of the small intestine, or inhibiting cytochrome P450 3A4 (CYP3A4) in the gut wall. Absorption was significantly increased by 7-10 mcg/mL/hour. The time to eliminate carbamazepine was also increased by 4-8 hours. Although carbamazepine levels were increased, this did not appear to increase side effects. In vitro research also shows that piperine can increase carbamazepine levels by 11% in a time-dependent manner.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, black pepper might decrease levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that black pepper induces CYP1A2. This has not been reported in humans.
Strontium
Androgens
Theoretically, androgens might reduce the excretion of strontium, potentially increasing its effects and adverse effects.
A small clinical study suggests that treatment with androgens such as testosterone, nandrolone, oxandrolone, and oxymetholone for 5-6 weeks might decrease urinary strontium excretion. The clinical significance of this interaction is unknown.
Antacids
Antacids can reduce the absorption of strontium, potentially decreasing its effectiveness.
Aluminum hydroxide and magnesium hydroxide can reduce absorption of strontium ranelate by 20% to 25%. Calcium can reduce its absorption to 60% to 70%. Avoid taking antacids within 2 hours of a dose of strontium ranelate. This interaction has also been reported with orally-administered radioactive strontium-85. It will likely also occur with other forms of strontium.
Estrogens
Theoretically, estrogens might reduce the excretion of strontium, potentially increasing its effects and adverse effects.
A small clinical study suggests that treatment with estrogens for 5-6 weeks might decrease urinary strontium excretion. The clinical significance of this interaction is unknown.
Quinolone Antibiotics
Theoretically, strontium might decrease the absorption of quinolone antibiotics.
Strontium is a divalent cation with similar properties to calcium. It is likely to form complexes with quinolones in the gastrointestinal (GI) tract, preventing absorption of the antibiotic. Recommend taking strontium supplements at least 2 hours before or after quinolones to avoid an interaction.
Tetracycline Antibiotics
Theoretically, strontium might decrease the absorption of tetracycline antibiotics.
Strontium is a divalent cation with similar properties to calcium. It is likely to form complexes with tetracyclines in the gastrointestinal (GI) tract, preventing absorption of the antibiotic. Recommend taking strontium supplements at least 2 hours before or after tetracyclines to avoid an interaction.
