Andrographis is generally well tolerated when used short-term. The main concern: long-term safety and product quality aren't well established.

Avoid it during pregnancy because of theoretical concerns and animal study findings about reproductive effects. There's also not enough safety data for breastfeeding, so skip it then too.

Common side effects include stomach discomfort, nausea, diarrhea, headache, fatigue, dizziness, rash, and itching. These are more likely at higher doses (5–10 mg/kg of the active component andrographolide) or treatment longer than 14 days.

Rare but serious reactions include severe allergies and anaphylaxis. Skin reactions — including Stevens-Johnson syndrome and other severe rashes — have been reported.

Theoretically, andrographis might decrease the absorption of etoricoxib, although the clinical significance is unclear.
Animal research shows that andrographis extract, or the constituent andrographolide, taken orally with etoricoxib decreases the bioavailability of etoricoxib. However, this reduced bioavailability is not correlated with a reduction in the anti-inflammatory effects of etoricoxib in arthritic mice models. The clinical significance of this interaction is unclear.
