Every active ingredient lists its own amount on the label.

Why this rating?
  • The label discloses an exact amount for 1 of its 1 active ingredient.
  • No proprietary blends here — you can verify the dose of every single component.

This product contains one active ingredient: black psyllium (plantain leaf), at 450 mg per capsule. Black psyllium is a soluble fiber that absorbs water and adds bulk to help move things through your digestive tract.

The capsule itself is made from gelatin and water — those are inactive ingredients that hold the psyllium together.

Strong evidence
Evidence for Intended Use · database check
By FDA rules, dietary supplements can’t claim to treat, cure, or prevent disease — so labels speak in careful marketing language. We discern each product’s intended use from its name, label claims, and label statements, then grade the clinical evidence for that use. How these ratings are computed

This product doesn't appear to be marketed for a specific use, so we graded Black Psyllium's overall clinical evidence instead.

Strong

Strong clinical evidence supports Black Psyllium for:

Why this rating?
  • We looked at the product name, claims, and label statements and couldn't find a stated purpose to grade.
  • Since the label doesn't commit to one use, we graded the ingredient's overall clinical evidence instead.
  • On file: Constipation — rated "Effective" (Natural Medicines).
  • On file: Coronary heart disease (CHD) — rated "Likely Effective" (Natural Medicines).

Black psyllium is effective for constipation and has been shown to help with coronary heart disease (CHD) — the evidence for heart health is strong. For obesity and diarrhea, we don't yet have reliable evidence to say whether it works, so those uses would require more research.

How safe is it?

Well-documented data
Safety Information · database check
Well characterized

Adverse-effect, pregnancy, and general safety data are on file for most of these ingredients.

Why this rating?
  • We hold adverse-effect (side-effect) data for 1 of the 1 matched ingredient.
  • Pregnancy & breastfeeding safety ratings cover 1 of 1.
  • General safety write-ups exist for 1 of 1.
  • Remember: this measures how much safety information exists. Thin data is not the same as being safe.

Black psyllium is generally well tolerated when you drink enough water with it — that's the key. The most common side effects are bloating and gas, which usually fade as your body adjusts and can be eased by increasing your dose slowly.

Serious problems like bowel or esophageal obstruction are rare but can happen if you don't take it with adequate fluids, so this isn't something to take dry. There's limited specific safety data on this product, so it's wise to check with your doctor or pharmacist before starting, especially if you have any digestive concerns.

If you've ever had an allergic reaction to psyllium products or dust exposure to psyllium (occupational or otherwise), avoid this — severe allergic reactions are possible in sensitized people.

Moderate identified

The most serious documented interaction for these ingredients is Moderate. Check your medications for a personalized result.

Why this rating?
  • 1 of the 1 matched ingredient can interact with medications — Black Psyllium.
  • The most serious interaction on file is rated Moderate.
  • Some involve high-stakes drug classes: heart-rhythm medications; lithium.
  • For scale: 2,026 individual medications appear in the full list. A big number alone doesn't make a product dangerous — what matters is whether YOUR medication is on it, so run yours through the interaction checker on this page.

Before taking this product, double-check with your pharmacist if you use metformin, lithium, carbamazepine, olanzapine, birth control pills, or digoxin. These Moderate and Minor interactions can change how well those drugs work.

Also, because psyllium can affect oral drug absorption generally, take it at least 30–60 minutes after any other medication.

The bottom line

Scorecard at a glanceFully disclosed formula with strong clinical evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.

This psyllium product works well for constipation and may support heart health. If you take metformin, lithium, carbamazepine, olanzapine, birth control, or digoxin, you'll need to time it carefully or check with us first — psyllium can interfere with how your body absorbs them.

Anyone with a history of psyllium allergy should skip it. Talk to your pharmacist before starting, especially if you have digestive issues or take any medications.

Each ingredient & the kinds of drugs it affects

For each ingredient in Plantain Leaf 450 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.

Plantain7 drug types · 2,025 drugs

Carbamazepine (Tegretol)

Theoretically, black psyllium might reduce the effects of carbamazepine and increase the risk for convulsions.
Theoretically, black psyllium might reduce carbamazepine absorption. A preliminary study using blond psyllium reported decreased carbamazepine bioavailability due to binding of the drug to psyllium, as well as reduction of available fluid in the gut for dissolution of the drug. This interaction may also occur with black psyllium.

Likelihood Probable Evidence D
Lithium

Theoretically, taking black psyllium at the same time as lithium might reduce lithium absorption.
The fiber in black psyllium might reduce lithium absorption and plasma levels. Some case reports describe a reduction in plasma lithium levels with concomitant administration of blond psyllium. This was reversed when psyllium was stopped. This interaction may also occur with black psyllium.

Likelihood Probable Evidence D
Metformin (Glucophage)

Theoretically, black psyllium might increase the therapeutic and adverse effects of metformin.
Animal research shows that concurrent consumption of blond psyllium with metformin slows and increases the absorption of metformin. This interaction may also occur with black psyllium. To avoid changes in absorption, take psyllium 30-60 minutes after metformin.

Likelihood Possible Evidence D
Olanzapine (Zyprexa)

Theoretically, taking black psyllium at the same time as olanzapine might reduce olanzapine absorption.
The fiber in black psyllium might decrease the absorption of olanzapine. A single case report describes a reduction in the effectiveness of olanzapine when it was concomitantly administered with an unspecified type of psyllium 3 grams orally twice daily. This effect was reversed when psyllium was stopped.

Likelihood Possible Evidence D
Digoxin (Lanoxin)

Theoretically, taking black psyllium at the same time as digoxin might reduce digoxin absorption and decrease digoxin levels.
Psyllium might bind digoxin in the gut. However, some clinical evidence suggests that psyllium does not impact digoxin absorption.

Likelihood Unlikely Evidence B
Ethinyl Estradiol

Theoretically, taking black psyllium at the same time as ethinyl estradiol might alter levels of estradiol.
Concurrent use of blond psyllium with ethinyl estradiol results in a slight increase in the extent of ethinyl estradiol absorption and a slower rate of absorption. This is unlikely to be clinically significant.

Likelihood Unlikely Evidence D
Oral Drugs

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.

Plantain Leaf 450 mg by TerraVita Premium Collection: Common Questions

Does Plantain Leaf 450 mg by TerraVita Premium Collection interact with any medications?
Yes. Based on its ingredients, Plantain Leaf 450 mg has a known interaction with 2,025 medications. Use the checker to see how it interacts with a specific drug.
How can one product interact with so many drugs?
Plantain Leaf 450 mg contains a single active ingredient, and that one ingredient can interact with many different medications on its own. We check it against each medication and show the mechanism responsible.
Where does this information come from?
The product label data comes from the NIH Dietary Supplement Label Database (DSLD); the interaction data is built on the Natural Medicines database and reviewed by HelloPharmacist pharmacists.
Will this help my constipation?
Yes — black psyllium is effective for constipation. Just make sure you drink plenty of water with it; without enough fluids, it can actually cause blockage instead of relieving it.
What are the most common side effects?
Bloating and gas are the most frequent, but they're usually temporary and get better as your body adjusts. Starting with a lower dose and increasing it gradually can help reduce these.
Is this safe during pregnancy or while breastfeeding?
The data suggests black psyllium is likely safe in pregnancy and lactation, but because there's limited specific safety information, it's still best to check with your doctor or midwife for your individual situation.
Can I take this at the same time as my other medications?
Not always. Psyllium works best if you wait 30–60 minutes after taking oral medications, since the fiber can bind drugs in your stomach and lower how much your body absorbs. Some medications have more serious interactions, so use the medication checker or ask your pharmacist about your specific prescriptions.
Is this product just fiber with no fillers?
The capsule itself is made from gelatin and water — those are the inactive ingredients that hold the psyllium together. So there are some other ingredients besides the active psyllium, but no extra fillers.
Can this help with heart health?
Black psyllium has been shown to be likely effective for coronary heart disease, so there is good evidence for that use. That said, talk to your doctor about whether it's right for your situation.
