Probiotic 123 contains four active ingredients, all live bacterial strains: Lactobacillus acidophilus (La-14), Bifidobacterium lactis (Bl-04), Bifidobacterium bifidum (Bb-06), and a Probiotic Blend. These are beneficial bacteria meant to support your gut flora.
The product also includes one inactive ingredient, hypoallergenic plant fiber, which acts as a filler.
Does it work?
Moderate evidence
Lactobacillus acidophilus is possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis. It's rated possibly ineffective for atopic disease (eczema, asthma, food allergies) and there isn't enough reliable evidence to rate it for acute respiratory distress syndrome (ARDS).
Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections, but evidence is insufficient to rate it for hay fever, Alzheimer disease, or generalized anxiety disorder. We don't have effectiveness data on file for Bifidobacterium lactis.
How safe is it?
Well-documented data
Lactobacillus acidophilus and Bifidobacterium bifidum are generally well tolerated in healthy people and children. That said, those with weakened immune systems should check with their doctor first, since live bacteria can rarely cause infection in vulnerable patients — cases of sepsis and other serious infections have been documented, though they're uncommon.
The most common side effects are mild gastrointestinal discomfort: bloating, flatulence, abdominal pain, or diarrhea; these are uncommon overall. Rash and itching have been reported in at least one trial, though it's unclear whether the probiotics themselves were responsible.
During pregnancy, both L. acidophilus and B. bifidum are rated possibly safe, but lactation data are not on file; talk with your doctor or pharmacist for personalized advice.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
