How safe is it?

Nattokinase is generally well tolerated in the short term, but its blood-thinning effect means you should be cautious if you have any bleeding risk or bleeding disorder. Rare reports of bleeding, including a documented case of brain hemorrhage, have been associated with it.
There's also a risk of allergic reaction in people sensitive to natto or nattokinase itself—symptoms like throat swelling can show up within 2 hours of taking it. Safety data during pregnancy is not sufficient to recommend use, so it's best avoided then.
The same applies to breastfeeding—safety is unknown, so it should be avoided.
