Nattokinase is generally well tolerated in the short term, but its blood-thinning effect means people with bleeding disorders or bleeding risk need to be cautious. Rare cases of bleeding, including serious brain bleeding, have been reported.

There's also a small risk of allergic reaction in people sensitive to natto or its components; symptoms can appear within two hours of taking it. There isn't enough safety data to recommend nattokinase in pregnancy, so it's best avoided then.

Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
