Here's the simple version: losartan (Cozaar) is a blood pressure medicine, and nattokinase may gently lower blood pressure on its own. Stack the two together and those effects can add up, so your pressure could dip lower than expected. This isn't about how your body breaks losartan down, it's just two things pushing in the same direction.
If your blood pressure drops too far, you might feel dizzy, lightheaded, tired, or a bit unsteady when you stand up quickly. To be fair, this concern is theoretical, based on how each works rather than strong studies, and the blood-pressure effect of nattokinase is mild. Still, I'd give your pharmacist or doctor a heads-up before adding it, and check your readings at home if you do.

Nattokinase is an enzyme from the fermented soybean food natto that may have mild blood-thinning and blood-flow effects. Some small studies hint at benefits for blood pressure and circulation, but the evidence is limited and more research is needed. What Is Losartan Used For?
Brand name: Cozaar
Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.
Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
