Nattokinase is generally well tolerated short-term, but because it has blood-thinning properties, it's not suitable for people at risk of bleeding. Rare cases of bleeding have been reported, including a serious one in someone who combined it with aspirin.

Do not use nattokinase during pregnancy — the safety data isn't there. Avoid it while breastfeeding as well.

Nattokinase is a unique enzyme created when soybeans are fermented to make natto. This plant-based enzyme derived from natto extract promotes healthy circulation throughout the body which supports the energy needs of the heart. Nattokinase 2,000 FU contains 100 mg (2,000 fibrin units) of nattokinase per capsule.
