Probiotic 50 Billion contains 11 active ingredients — a mix of live probiotic bacteria and prebiotic fibers. The bacterial strains include Lactobacillus salivarius, Lactobacillus plantarum, Lactobacillus bulgaricus, Bifidobacterium animalis lactis, Bifidobacterium longum, Lactobacillus rhamnosus, Lactobacillus acidophilus, Bifidobacterium bifidum, plus a proprietary Bifidobacterium blend.
The formula also includes a prebiotic blend and fructo-oligosaccharides — these are soluble fibers meant to feed the beneficial bacteria and help them thrive. The inactive ingredients are microcrystalline cellulose, a vegetarian capsule, silicon dioxide, magnesium stearate, and calcium silicate.

Evidence for most of these ingredients is still limited. Lactobacillus acidophilus shows possibly effective ratings for irritable bowel syndrome, antibiotic-associated diarrhea, bacterial vaginosis, and Helicobacter pylori infection — so there's some research backing its use for those specific conditions.
Bifidobacterium bifidum is possibly effective for irritable bowel syndrome and respiratory tract infections. For the other ingredients and conditions listed — like asthma, allergies, common cold, cognitive decline, and cholesterol — the evidence we hold is insufficient to rate either way.

How safe is it?
Well-documented data
The live bacteria in this product are generally well tolerated in healthy adults and children. However, people who are seriously ill or immunocompromised should be cautious — there have been rare cases of these bacteria causing bloodstream infections (bacteremia) or sepsis in critically ill or vulnerable patients.
The most common side effects from the probiotics and prebiotics are mild gastrointestinal ones: abdominal discomfort, bloating, flatulence, and constipation. These tend to be uncommon and mild, especially at lower doses.
For Bifidobacterium longum, a dry cough was reported by one person in a trial. Pregnancy safety data are limited — Lactobacillus acidophilus and Bifidobacterium bifidum are rated possibly safe in pregnancy, but the others lack enough study.
For breastfeeding, data are also limited and not well documented for most ingredients here, so check with your doctor or pharmacist.

Theoretically, taking Lactobacillus delbrueckii with antibiotic drugs might decrease the effectiveness of L. delbrueckii.
Lactobacillus delbrueckii preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. delbrueckii preparations by at least two hours.
Lactobacillus acidophilus
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium longum
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.

Arabinogalactan
Immunosuppressants
Theoretically, larch arabinogalactan might interfere with immunosuppression therapy due to immunostimulant effects. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and other drugs.
